We are recruiting an enthusiastic and innovative Allied Health Professional to join our community team as AHP Lead & Manager for the Living Well Service. This role offers a rewarding opportunity to help develop and deliver a range of person-centred interventions that support people with life-limiting illness to build self-management skills, take ownership of their lives and live as fully and meaningfully as possible. The post combines clinical practice with leadership and service development, managing a small team, delivering individual and group-based therapy programmes, and leading on community engagement to better understand local needs. Working closely with partners and supported by the Head of Community Palliative Care East, you will help shape and grow services that truly reflect what matters most to the people we support.
